Table 1

Plans we generally accept (see both tables)*:

  • Aetna commercial plans

  • Ambetter of Georgia commercial plans

  • Anthem / Blue Cross Blue Shield

    • Blue Cross Federal

    • HMO (with some exceptions - see table 2)

    • POS

    • PPO

  • Cigna

  • Humana (all except, "HMO-X" plans)

  • Kaiser Permanente (not accepting new patients)

  • Multiplan

  • PHCS

  • United Healthcare

Table 2

Plans we do not accept:

  • Aetna Medicare Advantage plans

  • Ambetter of Georgia Medicare Advantage plans

  • The following Anthem / Blue Cross products

    • Pathway-X plans

    • Guided Access plans

    • Medicare Advantage plans

  • Cigna Medicare Advantage plans

  • Humana "HMO-X" plans

  • Humana Medicare Advantage plans

     e.g., member IDs beginning with the letter "H"

  • Medicare, including Medicare Advantage Plans

  • Medicaid

  • Peach State Health Plan Medicaid plans

  • Tricare / Tricare Prime

  • United Healthcare Medicare

  • Wellstar employee health plan

     (you are required to see a Wellstar physician)

  • Worker's compensation plans (you need to see a provider on your employer's approved panel)

(*) Insurance networks are changing on a continual basis. It is your responsibility to verify with your insurer that we are in-network with your plan as insurers prohibit us from providing their members with benefits guidance.
